Title:                                    Social Worker (Assisted Living Supervisor)

  • The Social Worker functions as an integral member of the Líl̓wat Health and Healing team and applies best practice to manage and administer the Assisted Living program in accordance with the National Assisted Living Program Guidelines. sac-isc.gc.ca.

Key Deliverables:

  • Following the National Assisted Living Program guidelines develop a Líl̓wat Assisted Living service, including policies and procedures
  • Supervises homemakers and create the schedule for service delivery
  • Completes monthly, quarterly and annual internal/external reporting as required
  • Facilitates client and family meetings to address concerns and support the client’s well-being 

Key Qualifications and Attributes:

  • Bachelor’s Degree/Diploma in Social Work from an accredited University/College (Bachelor’s Degree preferred).
  • Minimum 1-year recent related experience in a complex health care environment such as a community or home health setting or a combination of education, training and experience
  • Demonstrated ability to assess clients at risk and knowledge of appropriate acts; e.g. Mental Health Act, Adult Guardianship Act, Continuing Care Act as it relates to the rights and obligations of the client/staff
  • Demonstrated knowledge and experience in providing culturally safe and competent care
  • Demonstrated experience in trauma-informed practice
  • Demonstrated ability to provide care planning, supportive counseling, crisis intervention, case coordination and establish workload priorities
  • Demonstrated ability to work independently and collaboratively as a member of an interdisciplinary team
  • Demonstrated ability to problem solve, use critical thinking skills, and deal effectively with conflict situations
  • Demonstrates effective and creative solution focused skills
  • Personable, with strong administrative skills e.g. budget management, staff supervision
  • Strong communication skills (verbal and written)
  • Strong computer skills e.g. MSOffice, MSWord, MSExcel
  • Strong understanding of the impacts of colonialism including residential schools, trauma, the breakdown in family systems and poverty
  • Training in stress management, time management, assertiveness, strategic planning and integrated case management are an asset
  • Must have a Valid BC Driver’s License
  • Must pass the Vulnerable Sector Criminal Record Check
